Improving Access to Chronic Disease Information and Education to Decrease Preventable Death in the Lower Mississippi Delta Sharkey Issaquena Community Hospital Sharkey Issaquena Community Hospital in Rolling Fork, MS, will add educational and informational outreach efforts to include chronic diseases of type 2 diabetes, hypertension, and cardiovascular disease. These efforts will concentrate on prevention through education, physical fitness, and healthy eating habits.
