Y5E - Pathways to Health Information: From Trusted Information to Informed Action

The Health Collaborative (THC) proposes Pathways to Health Information: From Trusted Information to Informed Action, a six-month health information literacy initiative embedded within six Pathways to Health Neighborhood Clinics and Mobile Community Health Worker Resource HUB deployments. At each Health Information Learning Center, bilingual Community Health Workers (CHWs), certified health insurance Navigators, and trained community partners will help residents use MedlinePlus and MedlinePlus en Español to understand preventive screening topics, medications, medical tests, and recommended next steps; identify trustworthy online health information; and connect information to coverage, primary care, specialty care, and evidence-based care coordination. The project will combine guided digital demonstrations, plain-language print and QR-code resources, a Trusted Health Information section within the participant Health Passport, and follow-up navigation after each clinic. Across six Learning Centers, THC will provide documented bilingual healthinformation education to at least 500 residents and complete at least 300 individualized MedlinePlus demonstrations. Through this approach, THC will transform one-time health information exposure into an accessible, culturally responsive pathway from screening to understanding, informed action, and connection to care
